Each request must contain the following:
- The requester's full name and address;
- A telephone number where the requester can be reached during business hours;
- A detailed description of the records requested, including relevant dates;
- The name of any pending litigation to which the request relates, the relevant court, and the court’s location; and
- The requester’s agreement to pay any fees related to the request, or a request for a waiver of such fees.
